What the current VFS materials say
VFS currently lists a notarized affidavit for change of appearance and signature among mandatory passport-reissue materials. Current adult reissue checklists also describe notarized color copies of valid U.S. status and proof-of-address documents in applicable cases. Minor-passport checklists can require notarized copies of both parents’ valid legal status.

Bring the exact form and checklist
Do not rely on an old photocopy of a checklist or a friend’s previous application. Bring the current official form, any affidavit that must be signed, your identification, and the supporting documents VFS says are required.
A North Carolina copy-certification issue to know about
VFS may use the phrase “notarized copy,” but North Carolina does not authorize a notary to certify a photocopy as a true copy. If the checklist calls for a notarized supporting copy, bring the exact instruction and any required affidavit or declaration so the notary can determine whether there is an eligible signature or notarial act to perform.

Call ahead and tell us roughly how many signatures or affidavits need notarization. The in-store fee is $10 per notarized principal signature. Witness service is $5 per witness signature when available.
